Im not sure that it's really a medical reason. Different Pediatricians have different opinions. Our Pediatrician told us to start solids at 4 months.

My first son has acid reflux and hard time gaining weight. The doctor told me go ahead start him on baby food. Then my second son my doctor said go ahead at 4 months old on baby food. Now my second daughter the doctor said they now wait till 6 months old. But we went to see a new doctor because my one I was seeing was no longer listening to my concerns. My daughter is failure to thrive and has acid reflux. They told me to start feeding her baby food so it would help her to gain weight.
My daughter is 19 months old but still not gaining weight like she should. She has a feeding tube is tube feed at night.

My son had to start on solids at 5 months. I exclusively breastfeed, but he stopped gaining weight. He was healthy and didn't look sick or anything, but in 2 months he'd only gained 1/2lb so my pediatrician wanted us to start giving him cereal for extra calories. I'd already tried taking meds to increase my supply, and he was draining my milk at every feeding, so based on his age this was her preferred approach. In the next month he gained 1lb 3oz :)

Like others said, GERD is a reason some docs start rice cereal earlier. It thickens the food and helps keep the baby from washing the food and acid back into their esophagus.

I disagree that breastfed babies don't need treatment for GERD. The actual diagnosis of GERD means that the baby fails to gain weight appropriately and is not a "happy spitter". It means they are miserable because of the acid that is burning their esophagus. Now, my son didn't need early solids, he started at 4 months just with the occassional taste, but he did need medication to help his GERD and still takes it at 8 months. Breastmilk was not enough to "cure" his acid issues.

Otherwise, solids are really the parents' concern. I wouldn't really listen to the doc if they said to start based on size. I never heard that any size is a reason to start solids early. Although babies can display readiness at different ages. Anywhere between 4 and 6 months is fine, at that stage it is still "additional" to the formula or breastmilk and shouldn't be considered a serious part of their diet. It is really not a staple until closer to a year (depending on the child depends on the age it is used nutritionally versus as an introduction).

I am not really sure. My pedi said between 4 & 6 months. I think 4 months is a little to early and I won't start them until 6 months.

I know with my nephew they had him on cereal in his bottle at about 3 months due to reflux..

babies that are on formula and develop GERD (gastric reflux) need to be put on rice cereal in order to thicken the formula and prevent the baby from regurgitating it into the lower esophagus and causing burns from the stomach acid.

other than that - none that i can think of.

if the baby's breastfed, btw, rice cereal is not necessary - the breast milk contains stem cells that buffer the stomach acid and help heal irritated tissues. my daughter was a reflux baby, exclusively breastfed. never needed treatment, just happily puked away until she was about 13mo when it all stopped - fast, too - i'm talking within two weeks, she went from firehose, to dribbler, to nothing at all.

pippin: i got that "too big" one, too!!!! this crackpot doc temping for my regular paed said that because my exsclusively breastfed daughter was 5mo old and already close to triple her weight (she was 7lb10 at birth, 32lbs at one year). i looked at him and said "so... what you're saying is that breastfeeding was enough to get her to this size... but somehow it's not enough any more!??

thankfully i had the sense to ignore this bafflingly moronic statement and continue my merry way and my daughter continued her merry weigh all the way up to 50lbs at 20mo when she suddenly stopped dead and is now starting to roll backward (she turned 3 in march and is down to about 47 - 48lbs).

If a baby had acid reflux they will start solids really early. My son started rice at 1 month and actual foods at 3 months. He had severe acid reflux and the solids helped him a lot with it. Another reason is when a baby constantly wants to drink formula they will tell you to try solids early if they are drinking over 32 oz a day. The formula has lots of calories the baby does not need when they drink that much.

usually baby rice is recommended before then for babies who suffer from acid reflux as it makes it thicker so its easier to keep down and helps ease the pain!

mum to 3.5yrold, 2.5yrold & 7.5 month old!

Premies sometimes run short on iron early, (they dont' have time to lay down sufficient stores en-utero). So, if iron supplements aren't appropriate or sufficient, then a premie may need solids earlier than typical.

Beyond that -- there's really no medical reason. But some doctors seem to believe that a 'big' baby needs solids early ("he's so big -- breastmilk obviously isn't enough for him anymore") and that 'small' babies need solids early ("she's so small, she obviously needs more calories to grow and thrive") and that 'average' babies need solids early ("he's so healthy and growing so well, there's no reason to delay solids.")

IOW, it's usually simple ignorance, or dated advice, or going along with mom's eagerness to start early.

EDIT: I wouldnt' consider rice cereal in a bottle for reflux to be 'starting solids.' It's medicine to treat a specific medical problem.

We started at 4 months by our doctors recommendation because our daughter had such bad acid reflux. His reasoning was that thicker substances will help to keep things down instead of coming back up and solid foods are thicker than milk.
